Application and registration
To Apply/Register – Contact AFHFL to request
an application. Register by mail or online at www.healthyfamily.org. Send your application
and deposit of $100 to confirm your place in
the training. For Credit Cards - Please see back page of the application or our website for details.
Tuition – Register early and save $45. Tuition is $550 when application and deposit of $100 are postmarked four weeks prior to the training, or $595 thereafter. The $100 deposit will be applied
to the total tuition. The balance of the tuition is due on or before the first morning of the training.

Policies:
-Purchase Order payments are not eligible for the early registration discount.
-Cancellation Policy - If you cancel less than 21 days prior to the training date, a fee of $100 is applied.
-If you are unable to attend the training in which you are already enrolled, and you choose to transfer to a future training of your choice, a $50 transfer fee will be applied.
-The trainer reserves the right to cancel the training if enrollment criteria are not met or when conditions prevail beyond her control. If we must cancel you will receive a full refund.
-In the event of incomplete attendance for any reason after the beginning of the course, all AFHFL training materials are returned to the organization. 0-50% of the tuition fee may be refunded at the discretion of the trainer.
-Return check fee is $25.
